This form is to be filled out and faxed or mailed to CUE statewide (FAX:
510/845-7444; address -- 2855 Telegraph Ave #302, Berkeley, CA 94705). DO
NOT forward electronically. A form must be submitted for every grievance
being appealed to arbitration. The Arbitration Appeals Panel meets
once a month. In order to be considered at that month's
meeting the form must arrive in the CUE statewide office 1 week before the
